Patient Zero is a dynamic zombie outbreak simulation for GTA V Enhanced. Start with a single infected NPC and watch the outbreak spread naturally through Los Santos as civilians panic, fight back, police respond and the military escalates the situation.

Sometimes the infection is contained quickly.

Sometimes the city completely falls apart.

Installation Guide:
Install Script Hook V.
Install Script Hook V .NET Enhanced.
Place PatientZero.3.cs inside your GTA V scripts folder.
Grand Theft Auto V

Launch Story Mode.
Press J to begin.

Controls

J – Start / Pause outbreak
F11 – Toggle cinematic camera
Page Up – Infect nearest healthy NPC
Page Down – Reset the outbreak

Features

• Dynamic Infection

Every outbreak begins with Patient Zero.

The infected hunt nearby civilians and spread the virus from person to person. Because NPCs, traffic and locations are constantly changing, the outbreak develops differently every time.

Start the infection anywhere in Los Santos and see what happens.

Escalating Response

The city reacts as the infection grows:

20 Infected – Civilian Resistance
25 Infected – Special Infected Begin Appearing
30 Infected – Police Response

What starts as one infected civilian can eventually turn into a full-scale emergency.

Special Infected

As the virus mutates, stronger infected can appear:

Sprinter – Faster and more aggressive
Brute – Stronger and much harder to kill
Screamer – Creates additional chaos during larger outbreaks

Civilian Panic & Traffic Chaos

Civilians may flee, become trapped in traffic, abandon vehicles or fight back.

This creates unpredictable situations where civilians, infected, police and military forces can all collide at once.

Police, NOOSE & Air Support

As the outbreak escalates, increasingly powerful forces attempt to contain it.

Expect police, armed civilians, military soldiers, tanks, NOOSE teams, jets and heavy-response events.

No Two Outbreaks Are the Same

Patient Zero is built around emergent gameplay.

Different people get infected.
Different civilians fight back.
Traffic changes.
Special infected appear in different situations.
Police and military encounters develop differently.

One outbreak may collapse almost immediately.

Another may reach 100 infected and overwhelm the city.
